Final Clearance - Doctoral Level
FORM IV is the Final
Clearance. This form is to be completed and filed prior to
Commencement. The due dates are available at
the Division of
Graduate Studies Website. (Click here for dates).
To expedite your clearance by the Division of Graduate Studies, you
should bring with you:
- A current degree evaluation completed with your advisor
- Your copy of the Application for Graduate Degree Candidacy
- Notice of Passing the Area Comprehensive
Examination.
- A copy of the "Committee Report of Defense Results"!

this form with your advisor!
Now is the last time to
conduct a degree evaluation with your advisor. You must have
completed the courses listed with a “B” or better or be currently
enrolled in the classes. You may
need to revise your plan of study or a Course
Substitution Form.
Checklist
- All course work completed or currently
enrolled in classes.
- All incomplete grades cleared.
- Area Comprehensive Examination passed.
- Committee Report of Defense Results on file in the Division of Graduate Studies.
- All departmental requirements completed.
- Cap and gown ordered for Commencement.
- Obtain information on
Commencement Activities from the JSU
Website.

- Obtain the signatures of................

- Your Department Chair/Program Director
- Your College Dean
- The Dean of the Division of Graduate Studies
- The Office of Financial Services* (There is a $75.00 dissertation binding
fee, payable in cash only)
- The Registrar
