\n\t·         Lowered closet rods<\/span><\/span>\n<\/p>\n

\n\t·         Main-level room and\/or avoidance of stairs<\/span><\/span>\n<\/p>\n

\n\t·         Wheelchair access to elevator<\/span><\/span>\n<\/p>\n

\n\t·         Wheelchair-accessible furnishings<\/span><\/span>\n<\/p>\n

\n\tMaking a request<\/strong><\/span><\/span>\n<\/p>\n

\n\t·         Establish an Accommodation Plan with the Disability Center.<\/span><\/span>\n<\/p>\n

\n\t·         Notify Disability Services for Housing Accommodations as early as possible.<\/span><\/span>\n<\/p>\n

\n\t·         Provide supporting documentation and an evaluation by an appropriate professional that                   relates the current impact of the disability to the request.<\/span><\/span>\n<\/p>\n

\n\tDocumentation should include:<\/strong><\/span><\/span>
\n\t·         A diagnostic statement including the date of the most recent evaluation<\/span>\n<\/p>\n

\n\t·         A description of the diagnostic criteria or tests used<\/span><\/span>\n<\/p>\n

\n\t·         A statement about the current impact of (or limitations posed by) the disability<\/span><\/span>\n<\/p>\n

\n\t·         A statement about the expected duration, stability or progression of the disability<\/span><\/span>\n<\/p>\n

\n\t·         The credentials of the diagnosing professional<\/span><\/span>\n<\/p>\n

\n\t·         If possible, submit recommendations from the diagnosing\/treating professional. <\/span><\/span>\n<\/p>\n

\n\tRecommendations should:<\/span><\/span><\/strong>\n<\/p>\n

\n\t·         Provide a clear description of the recommended housing accommodation<\/span><\/span>\n<\/p>\n

\n\t·         Connect the recommendation to the impact of the disability<\/span><\/span>\n<\/p>\n

\n\t·         Suggest possible alternatives to the recommendation<\/span><\/span>\n<\/p>\n

\n\t·         Include a statement of the level of need for (or the consequences of not receiving) the                          recommended configuration<\/span><\/span>\n<\/p>\n

\n\t·         Submit requests for housing accommodations as early as possible for priority consideration.<\/span><\/span>\n<\/p>\n
