The check should be made to Department of State \u2013 for $110.<\/p>\n<p><em>2)\u00a0\u00a0\u00a0\u00a0<\/em><em>Evidence of U.S. citizenship<\/em><\/p>\n<p><em>(previous undamaged, fully valid U.S. passport; certified U.S. birth certificate; Consular Report of Birth Abroad; Naturalization Certificate)\u00a0<strong><u>Original and a photo copy <\/u><\/strong><\/em><\/p>\n<p><em><strong><u>(<\/u><\/strong><\/em><em><strong><u>Please note that this item, both original and copy, will have to accompany your passport application.
