{"id":34,"date":"2014-09-23T16:35:09","date_gmt":"2014-09-23T16:35:09","guid":{"rendered":"https:\/\/www.jsums.edu\/healthservices\/?page_id=34"},"modified":"2025-04-08T14:17:56","modified_gmt":"2025-04-08T14:17:56","slug":"immunization-requirements","status":"publish","type":"page","link":"https:\/\/www.jsums.edu\/healthservices\/immunization-requirements\/","title":{"rendered":"Immunization Requirements"},"content":{"rendered":"<p><a href=\"https:\/\/www.jsums.edu\/healthservices\/jackson-state-university-immunization-policy\/\">Immunization Policy Details<\/a>\u00a0| <a href=\"https:\/\/www.jsums.edu\/slpaud\/files\/2022\/06\/IMMUNIZATION-DOCUMENTATION-FORM.pdf\">JSU Immunization Form<\/a><\/p>\n<p>All students admitted to Jackson State University \u00a0 are required to submit proof of having received two immunization shots for measles, mumps, and rubella (MMR) in order to complete registration for classes.<\/p>\n<p>If an applicant has already been admitted and plans to attend the University as a first time student (undergraduate, graduate or transfer), he\/she must submit a copy of the required immunization record to the University Health Center prior to registration for courses.<\/p>\n<p>Students who have been admitted but do not submit the required immunization verification will be unable to complete registration for classes in the school\u2019s computer enrollment system.<\/p>\n<p>The University Health Center will accept any immunization document that shows proof that the student has had the two MMR shots (measles, mumps, rubella) and has the same identifying information required on most immunization compliance forms (see JSU Immunization Form link on web page)<\/p>\n<ul>\n<li>Your name, address, date of birth<\/li>\n<li>Dates that the two MMR shots were given (month, day, year)<\/li>\n<li>Name, address and phone number of verifying health care facility or provider<\/li>\n<\/ul>\n<p>&nbsp;<\/p>\n<p>Applicants can also download and print the JSU Compliance Form and have their healthcare provider complete and sign it or use the Mississippi Form 121 or their own state\u2019s standard compliance form.<br \/>\nAll immunization documents can be submitted in one the following ways:<\/p>\n<p><span style=\"line-height: 1.6em;\"><strong>FAX TO:<\/strong> \u00a0 601-979-9228\u00a0\u00a0<\/span><\/p>\n<p><span style=\"line-height: 1.6em;\">Or\u00a0\u00a0<\/span><\/p>\n<p><strong>Email: \u00a0<\/strong><br \/>\n<span style=\"line-height: 1.6em;\">healthservices@jsums.edu<\/span><\/p>\n<p>Or<\/p>\n<p><strong>IN PERSON:<\/strong> \u00a0Immunization documents may be dropped off to the University Health Center located on Prentiss Street near the Prentiss Street entrance (FLAT TOP ONE STORY BUILDING WITH BLUE TRIM AROUND THE TOP, RECTANGULAR SHAPE).<\/p>\n<p>(For more detailed information about JSU\u2019s immunization policy including exemptions, alternative blood test as proof, immunization forms etc, go to \u201cImmunization Policy Details\u201d link above.)<\/p>\n<p>For questions, the Health Center can be contacted by email at <a href=\"mailto:healthservices@jsums.edu\">healthservices@jsums.edu<\/a><\/p>\n","protected":false},"excerpt":{"rendered":"<p>Immunization Policy Details\u00a0| JSU Immunization Form All students admitted to Jackson State University \u00a0 are required to submit proof of having received two immunization shots for measles, mumps, and rubella (MMR) in order to complete registration for classes.
