The Plan offers two coverage choices for active employees, COBRA participants
\nand non-Medicare eligible retirees: Base Coverage and Select Coverage. Each
\ncoverage type is independent of the other.
\nAn employee may choose Base Coverage or Select Coverage at initial enrollment.
\nIf timely application is made and the appropriate premium amount is paid, the
\neffective date of coverage for the employee and any eligible dependent(s) begins
\nthe first day of employment. If an employee does not enroll or if he waives
\ncoverage within 31 days of employment, application may be made only during an
\nopen enrollment or special enrollment period.<\/p>\n
A 2021 Plan Document (PD), which contains benefits, plan provisions, and
\neligibility guidelines, will be available on the plan Website. You can find an
\nelectronic version of the PD online at http:\/\/knowyourbenefits.dfa.state.ms.us
\nunder Publications. If you do not have access to the internet, you may
\nrequest a paper copy of the PD by calling the Office of Insurance, (601)359-3411,
