{"id":1833,"date":"2016-03-14T21:08:49","date_gmt":"2016-03-14T21:08:49","guid":{"rendered":"https:\/\/www.jsums.edu\/jsucorc\/?page_id=1833"},"modified":"2018-03-14T09:59:31","modified_gmt":"2018-03-14T09:59:31","slug":"community-health-advisors-network-chan","status":"publish","type":"page","link":"https:\/\/www.jsums.edu\/jsucorc\/community-health-advisors-network-chan\/","title":{"rendered":"Community Health Advisors Network (CHAN)"},"content":{"rendered":"<p>\n\t&nbsp;\n<\/p>\n<p>\n\t<a href=\"https:\/\/www.jsums.edu\/jsucorc\/files\/2016\/03\/CHA-members.jpg\"><img decoding=\"async\" loading=\"lazy\" alt=\"\" class=\"alignnone size-full wp-image-3152\" height=\"375\" src=\"https:\/\/www.jsums.edu\/jsucorc\/files\/2016\/03\/CHA-members.jpg\" width=\"662\" srcset=\"https:\/\/www.jsums.edu\/jsucorc\/files\/2016\/03\/CHA-members.jpg 662w, https:\/\/www.jsums.edu\/jsucorc\/files\/2016\/03\/CHA-members-300x170.jpg 300w\" sizes=\"(max-width: 662px) 100vw, 662px\" \/><\/a>\n<\/p>\n<h4>\n\tPurpose:&nbsp;<br \/>\n<\/h4>\n<p style=\"text-align: justify\">\n\tThe Community Health Advisors Network (CHAN)&nbsp;supports, promotes, and links local people who are natural helpers&nbsp;to improve individual, family and community health.<br \/>\n\t&nbsp;\n<\/p>\n<h4>\n\tCHAN Mission:&nbsp;<br \/>\n<\/h4>\n<p style=\"text-align: justify\">\n\tThe mission is to provide information, training, technical assistance and linkage to sponsoring organizations of Community Health Advisors programs and to take a leadership role in community-driven health promotion.&nbsp;<br \/>\n\t&nbsp;\n<\/p>\n<h4>\n\tCore Values:&nbsp;<br \/>\n<\/h4>\n<p style=\"text-align: justify\">\n\tThe Community Health Advisors&nbsp;Networks are the hearts and hands for the health of their communities. &nbsp;The CHAN focuses on local communities&nbsp;to help expand opportunities&nbsp;to pursue the best health possible through education, relationship building, and the exchange of ideas that spark living healthier, happier&nbsp;and longer. &nbsp;\n<\/p>\n<ul>\n<li>\n\t\tCommunities are capable of identifying their needs, setting agendas, developing solutions and making needed changes.&nbsp;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tCommunity-driven change creates ownership and is the most appropriate, effective, and lasting way to facilitate improvements in health.<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tNurturing positive community change required long-term commitment and adaptability.<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tHolistic community&nbsp;health promotion improves the quality of life.<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tSharing sound health and leadership information improves health outcomes.<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tEmpowered natural helpers are key to effective change. &nbsp;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<\/ul>\n<hr \/>\n<p>\n\t&nbsp;\n<\/p>\n<p style=\"text-align: justify\">\n\tThe Jackson Heart Study Community Health Advisors&nbsp;(CHAs) are engaged in developing and implementing health and wellness projects throughout the cohort community of Hinds, Madison, and Rankin counties.&nbsp; The CHAs operate as agents of the Community Health Advisor&nbsp;Network (CHAN) program, and currently there are five CHANs located in the JHS area; Jackson, Clinton, Bolton\/Edwards, Rankin, and Canton. The goals of the CHAN are:\n<\/p>\n<ul>\n<li>\n\t\tTo promote awareness of Jackson Heart Study initiative;&nbsp;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tTo inform the lay community of Jackson Heart Study findings;&nbsp;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tTo provide culturally appropriate heart health information to the community;&nbsp;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tTo provide venues for health screenings, exercise demonstrations, and health care information exchanges;&nbsp;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tTo increase the number of community agencies, churches, and health care providers who are Jackson Heart Study partners;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tTo introduce healthy lifestyle choices to the public and;&nbsp;<br \/>\n\t\t&nbsp;\n\t<\/li>\n<li>\n\t\tTo provide leadership training to Community Health Advisors (CHAs) to facilitate financial independence and sustainability.\n\t<\/li>\n<\/ul>\n<p>\n\t&nbsp;\n<\/p>\n<p style=\"text-align: justify\">\n\t<span>The staff of the Community Outreach Center sponsors leadership development trainings for the Community Health Advisors to increase CHA awareness and use of community resources and encourages CHAN sustainability. CHAs have received training in areas such as grant-writing, research, establishing a 501 C 3 nonprofit organization and diabetes overview and glucose screening techniques.
