Required Documents
To use your VA Educational Benefits at Jackson State University, you must submit the required documentation listed below. Enrollment cannot be certified with the Department of Veterans Affairs until all required documents have been received.
Semester Certification Requirement
Veterans & Military Center Intake Form
The Intake Form must be submitted each semester you intend to use your VA Educational Benefits. This form notifies the Veterans Center of your intent to utilize your benefits.
Community Service
- Military service may qualify a student for 60 community service hours toward the University’s 120-hour community service graduation requirement. The student remains responsible for completing the remaining 60 community service hours to satisfy the graduation requirement. Military Service Documentation
Submit the documentation that applies to your military status:
Veterans
- DD Form 214 (Member 4 Copy)
National Guard & Reserve Members
- DD Form 2384 (Notice of Basic Eligibility – NOBE)
- Current Orders
- Drilling Schedule/Dates (Obtain from your unit)
Important: If you are deployed during the semester, you must provide a copy of your official orders to both your instructors and the Veterans Center.

Current Curriculum/Degree Plan
A Certificate of Eligibility must be on file before enrollment can be submitted to the VA.
VA Education Benefit Applications
The Department of Veterans Affairs must have the appropriate application on file for your benefit type.

Veterans
- VA Form 22-1990 – Application for Education Benefits
- VA Form 22-1995 – Request for Change of Program or Place of Training
Dependents & Spouses
- VA Form 22-1990E – Application for Transfer of Entitlement (TOE)
- VA Form 22-5490 – Application for Survivors’ and Dependents’ Educational Assistance
- VA Form 22-5495 – Request for Change of Program or Place of Training (DEA Recipients)
Scholarship Reporting Requirement
Students receiving scholarships must notify the Veterans Center.
Examples include:
- Tuition Scholarships
- Fee Waivers
- Book Scholarships
- Other Educational Assistance Awards
Scholarship information must be submitted to our office for VA reporting purposes.
Change of Program or Place of Training
Students who change their major must submit an updated VA form:
- VA Form 22-1995 (Veterans)
- VA Form 22-5495 (Dependents/DEA Recipients)
Failure to submit the appropriate form may delay certification of your benefits.
